"Aging and Disability Resource Center"
An Open Door, No Wrong Door Entry into Long Term Support Services
The Aging and Disability Resource Center (ADRC) provides citizen centered “one-stop” entry points into the long term support (LTS) system. We serve individuals in need of LTS, caregivers and those planning for future LTS needs. This program was launched in the Fall of 2006 and is a collaborative effort of various agencies statewide. The key to the ADRC is teh many partnerships that exist between us and these various other agencies.
The ADRC maintains an open door policy to assist individuals in getting the care they require to maintain the quality of life they desire. We strive to achieve this by offering three main functions.
Awareness and Information
- Public Education
- Information on LTS options
- LTS options counseling
- Benefits counseling
- Employment options counseling
- Referral to other programs and benefits that can help people to remain in the community, including programs to assist in obtaining and sustaining paid employment
- Crisis intervention
- Assistance in planning for future LTS needs
Access
- Eligibility screening
- Assistance in gaining access to LTS services that may be paid with private funds
- Comprehensive assessment of LTS needs and care planning
- Programmatic eligibility determination for LTS services, including the Medicaid long term care level of care determination
- Facilitation of Medicaid financial eligibility determination that is either integrated or so closely coordinated with the ADRC that applicants experience a seamless interaction
- One stop access to all public programs for community and institutional LTS services administered by the state under Medicaid, Older Americans Act programs devoted to LTS services and any other publicly funded services the state determines should be accessed through the ADRCs.
